Hemorrhoids or Anal Fissure

Im on my 3rd trimester na (28weeks) mga momsh and always constipated kahit dami na fluids intake. May times pa na sa super hard may kasama ng blood sa stool pero walang pain, may times din na ok naman. When is the best time to consult the dr?
