How do you correct misspelled name in your birth certificate in the Philippines?

Hi. I hope this helps. The wrongly spelled name in the birth certificate should be corrected by filing a petition for correction of clerical error under the provisions of Republic Act 9048. The owner of the record and or authorized person by the owner can do the filing. If minor, petition may be filed by parents, brothers, sisters or guardians. The petition shall be filed with the local civil registry office of the city or municipality where the birth is registered. The supporting docs are certified machine copy of the birth record containing the entry to be corrected. Notice/certificate of posting. Payment of P1,000 as filing fee. Not less than 2 private or public documents upon which the correction shall be based like baptismal certificate, business record, bank passbook or NBI/Police clearance.
