hi! ask ko lang what if hindi pa kayo kasal ni hubby anong documents ang need dalhin para apelido nya ilagay sa birthcert ni baby. ps. may i know also what kind of documents we need to bring pag ka panganak. thankyou.

may affidavit lang na ssignan ung father sa likod ng certificate of live birth na pprovide ng hospital po.. tpos ipprocess yta sa city/municipal hall..
