PTPA To those who are asking paano mag submit or notify ng MAT1 sa SSS thru online. Hopefully this may help you lalo na mahirap at bawal talagang lumabas ang preggy mommy as of the moment. 1. Go to sss.gov.ph. 2. Click member and submit your user name and password. 3. Click inquiry, then sickness / maternity. Click again the sickness and maternity claim info. 4. Click maternity notification and answer necessary details needed. NOTE: - EDD lang yung need nila. - Then IPADALA (bawal kang pumunta sa office nila) sa pinakamalapit na SSS office yung form ng MAT1 and photo copy ng first ultrasound. - Ilalagay sa brown envelop indicated sa likod all capitalized Name and SSS Number and also MAT1 or MATERNITY BENEFIT (Depende to sa guard o kung sino man mag aassist sa inyo) - Then iiwan sa drop box for maternity benefit. Wait for the text ni SSS kung kailan ka papapuntahin for any clarification or ok na yung MAT1 nyo.
