Ano requirements pra maka avail ng Philhealth? My ultrasound result, birth certificate at Id na po ko, ano p po kulang? Need pag ba tlaga ng medical certificate?

Thanks momsh kaya nga babalik ako dun now eh kasi pinapakuha ako ng medical certificate as a proof eh ayan na ung ultrasound copy ko.
