Philhealth Maternity Requirements

Bago po ung mdr if nagbago ng employer, mababago din Philhealth employer ID and details nung employer sa babang part ng MDR. If naging voluntary nmn, mababago ung membership category and wala ng details ng employer. Kung ipapalakad namn ung requirements, need ng authorization letter with 2 valid ids mo and your representative. Make sure to photocopy yong IDs.
