Samin mamsh HR ang nag process. If I remember correctly, PSA copy ng birth certifcate ni baby kailangan. Verify mo na lang rin sa SSS website or sa HR nyo.

if employee usually bibigyan ka ng list of requirements ng hr. certified true copy from city/ municipal hall na birth cert pwede naman.
