Mat2 form fully accomplished with signature of employer and attached Mat1 form, live birth ni baby, hospital discharge form and photocopy of i.d's.

pwede po kaya kahit statement of account lang ang ipasa kasama ng mat1 confirmation,mat2 form and bc ni baby kapag cs?
