Lost of health booklet

Can check with the hospital your child is born, because I noticed babies' health booklet is issued by the hospital and (it bears the name of the hospital) they have the babies info during birth and etc. Can also check with polyclinic if not, you can call MOH hotline and ask.

Ive read before that u can contact the hospital ur child was born and have to pay a replacement fee

So sorry for the ordeal u hv to go thru….hope everything is better already

I think can either call MOH hotline or the hospital the baby was born at.
