HELP! Toddler Ripped His Vaccination Chart and Dental Chart.

HELP!! Team BakuNanays, please help me my toddler accidentally ripped his only vaccination card and dental chart to pieces. Is there a way that I can get another one? I worry that without it he may not be able to apply for school, as I am aware that it is part of the requirements. Also will he need be vaccinated again like going back to square 1? Any suggestions that will help me? Please comment it down.

aww. if from pedia, i think it'll be easier to get copies, if from brgy center, try to ask them nicely if you can secure another one as they have to go yhrough their big book to check your LO's record. it may be helpful if you know the dates.
