Redness on LO

Hi mummies can someone help to tell me what is this? Is it call heat rash or ezcema or other names? this redness is getting bad and been 4 momths already. How to get rid of this?

Usually rash is on the entire area. Could have been insect bite but baby keeps scratching so it got worse? Best to see doctor if it has been there for so long. At least get some cream for relief. Totally unrelated but my bub has the same singlet😄
