certified true copy from municipal office is good basta may tatak. or in my case kasi the certificate from the hospital na registered na din sa munisipyo ang sinubmit ko. they accepted it naman.

if submitted na yan ng hospital at may registration number na dapat ok na. makakakuha ka na ng CTC nyan from them.
