philhealth concern sa panganganak

Married po ako pero hiwalay na kami. Ngayon po, im pregnat with my new partner, what i am worried po is ginagamit ko na kasing surname ko sa checkups ko is ung maiden name ko pero ung philhealth ko is updated sa surname ng then-husband ko. Should i use ung married name ko para maClaim ung philhealth benefits pag nanganak ako or ituloy ko na lang ung paggamit ng maiden name ko. And isa pa po, pag sa birth cert ng baby ko, ano din ang ilalagay ko. Please dont judge, i just need po honest answer thank you.
