This is a child vaccination package recommended under the National Childhood Immunisation Schedule, for children under 18 months of age. With the necessary information and documentation required, our medical team will administer the vaccines directly in your home.

Terms & Conditions:
- Only applicable for Singapore citizens only and for children under 18 months of age.
- Once you have submitted the completed form, you will be contacted by a mobile health assistant within 2 working days to confirm your appointment details as well as the type of vaccines required by your child as per the National Childhood Immunisation Schedule.
- Our mobile medical team will arrive at your preferred address and timing to perform the vaccination. Based on the schedule of the vaccination required, a Child Development Assessment conducted by a doctor will be arranged if needed.
- We require full disclosure of the child’s immunisation records before confirming the vaccine purchase and appointments. You must provide complete and up-to-date immunisation records to ensure your child’s eligibility for vaccination. Failure to provide accurate and complete records may result in a delay or cancellation of the vaccination appointment. There will be no refunds for such cancellation. We strongly recommend that you consult our doctors on the DA app or a medical professional for clarifications on your suitability, or your family member’s suitability, for vaccinations.
